Results showed that diet quality, defined by the presence of whole grains, fruits, vegetables, nuts, and legumes, was the primary determinant of health outcomes, with processing levels having minimal independent impact.

Xiaowen Wang, a postdoctoral research fellow at Harvard T.H. Chan School of Public Health and co-author of the study, stressed that while ultra-processed foods like fortified cereals or canned beans can contribute to a healthy diet, their nutritional value depends on the broader context of a person’s eating habits. “A healthy plant-based diet rich in whole grains, fruits, vegetables, nuts, legumes and healthy plant oils is beneficial, even if it includes some processed foods,” she said. The research also highlighted that many ultra-processed items, such as fast food or sugary snacks, are often linked to poor dietary quality due to high levels of added sugars and refined carbohydrates.

The study’s methodology separated diet quality from processing levels, using the Nova classification system to evaluate food categories. Participants were grouped based on their plant-based eating patterns, with examples of healthy, ultra-processed options including whole grain cereals with fortified milk and plant-based yogurt. Unhealthy, minimally processed items like white rice or fruit juice were also categorized. Over time, about half of the participants developed heart disease, Type 2 diabetes, or died, but the study found no significant differences in risk based solely on processing levels.

The Harvard study’s emphasis on diet quality over processing levels could influence public health messaging, particularly as consumers navigate complex food environments. Wang noted that while ultra-processed foods should generally be limited, their role in a balanced diet depends on their nutrient profile. “Processing itself is not inherently harmful,” she said, advocating for a focus on overall dietary patterns rather than rigid distinctions between processed and unprocessed foods.

Meanwhile, research on mental exhaustion from Johns Hopkins University suggests that cognitive fatigue can alter decision-making, potentially affecting food choices. However, the Harvard study did not address this aspect, focusing instead on long-term health outcomes. Together, these findings highlight the interplay between individual habits, systemic challenges like foodborne illness outbreaks, and the need for nuanced approaches to nutrition.
